Abbreviation Pitfalls

Abbreviations save time but create ambiguity (BS = blood sugar or breath sounds?). Prefer clear wording for high-stakes concepts. If your agency requires abbreviations, still expand critical items once.

Watchlist examples: SOB, CP, LOC, AMS, WNL, NAD, AMA.
