Excellent DCHART

DCHART is Dispatch + CHART. Many FTOs and QA desks mark this even when the policy PDF says CHART. Dispatch is not a throwaway line — it is how you prove response, scene, and the position you found.

D — Dispatch / arrival

Excellent D names the unit, dispatch time, nature of the call, on-scene time, location type, hazards, who was already there (engine, PD, family), and position found. Example: Medic 12 / Engine 4 dispatched 09:12 for 58-year-old male chest pain after shoveling, spouse caller, Priority 1. On scene 09:14, packed snow, patient sitting in a living-room chair, pale and diaphoretic, clutching his chest. No scene hazards. BLS first-in; ALS one minute later.

D is Objective-adjacent: what you walked into, not the full OPQRST. Save the quote and OPQRST for C and H.

Then write CHART as usual

C: crushing substernal pain after shoveling — quote the elephant line.

H: OPQRST, stent history, aspirin 81 not taken, denied SOB at onset, no syncope.

A: diaphoretic, lungs clear, serial vitals 09:16 and 09:28.

Rx: ASA 324 mg chewed 09:22; NTG 0.4 mg SL 09:25; pain 8→5; SBP stayed above 100.

T: ALS to PCI-capable ED 09:32–09:44, monitored, bedside report, spouse POV. Necessity: ACS picture, required monitoring, not a private-vehicle ride.

Why D changes the grade

Without D, a CHART note can look like the call began at the chair. With D, a reviewer sees response interval, first-in vs ALS, winter scene, and that the patient walked inside with a spouse — mechanism and timeline before you touched them.

DCHART failure modes

• D is only "dispatched chest pain."
• Repeating the entire H inside D.
• Forgetting position found (chair vs floor vs still in the snow).
• No on-scene clock time.
• Skipping D entirely because the studio outline said CHART — if you chose DCHART, lead with D.
