Medical Necessity Language

Medical necessity documentation explains why ambulance transport was required—clinical condition, monitoring needs, inability to safely go by other means—not just that a transport occurred.

Avoid "routine transport" and "transported without incident" as the whole story. Tie findings to why ALS/BLS monitoring or positioning mattered. This lesson is educational, not billing advice.
