Objective Language

Write what you saw, heard, measured, and what the patient said. Avoid labels ("crazy," "drug seeking," "noncompliant"). For behavioral health, describe behavior, speech, affect, risk statements, and safety actions.

Objective ≠ cold. You can be precise and respectful.

Good snippet

Patient pacing, speaking rapidly, stating "everyone is watching me." Oriented to person/place; not oriented to date. No weapons visible. Agreed to voluntary transport after options explained.

Weak snippet

Psych patient acting crazy and noncompliant. Drug seeking.
