Objective Language
Write what you saw, heard, measured, and what the patient said. Avoid labels ("crazy," "drug seeking," "noncompliant"). For behavioral health, describe behavior, speech, affect, risk statements, and safety actions.
Objective ≠ cold. You can be precise and respectful.
